Terms and Conditions Young Functional Nutrition, Young Holistic Health and/or Nick Young, views health and well being in a holistic manner. Consultations include a comprehensive intake that carefully evaluates the client's health complaints on the mental/emotional, and physical level. Certified laboratory screenings may be recommended by your practitioner. Any/all lab testing that may be performed is NOT used to diagnose/label the client with an illness or disease; rather, these laboratory screenings are used to evaluate the function of various organs, systems and glands within the body. It is common practice and within Safe Harbor/Health Freedom laws, that a non-licensed practitioner may offer and/or provide consulting services to a client. Nick Young is a board-certified Functional Diagnostic Nutrition Practitioner and Holistic Health Practitioner.

 

Clients may be asked about their temperament, personal habits, likes/dislikes and unique outlook on life. Providing this information will allow the practitioner to understand each client as an individual and to provide the most appropriate means of care. This view differs from most conventional approaches, which typically limit concerns to the individual symptoms and their care. The goal of Young Holistic Health's practice is to strengthen the constitution of the whole person, which may result in alleviation of symptoms and an overall increase in health; however, there is no guarantee of these outcomes.

 

All client information collected by the practitioner, whether communicated verbally or written by the client, is to remain confidential and in accordance with HIPAA. The client agrees that he or she will receive a nutritional and functional interpretation of lab screening results (if any; this service is optional), that are to be used exclusively by the client, as an educational tool for personal health purposes. However, the personal physician of the client can use these same laboratory results to diagnose and/or treat disease.

 

The information on Nick Young’s web sites, social media, flyers, brochures and information packets are believed to be extremely accurate, but such accuracy cannot be guaranteed as we are not the originators of all the underlying data used in the interpretation.

 

LIABILITY WAIVER:  The undersigned also releases Nick Young from any liability for any injury or loss arising out of the use of, or reliance on, the laboratory results and/or the dietary, supplement and lifestyle suggestions provided. Before making any changes to the exercise, diet, or nutritional or hormonal augmentation of the undersigned, a physician should be consulted. The undersigned agrees that it is his or her responsibility to deliver all laboratory test results, now and in the future, to his or her own physician for any medical interpretation or opinion regarding any laboratory results provided by Nick Young. Nick Young does not diagnose, cure or treat any illness or disease. Out of Laboratory Reference Range results will be indicated on the original Official Lab Result form provided by a State Certified Laboratory to the undersigned. This information is not intended to, cannot, and should not be expected to, substitute for a personal consultation with your own physician. Further, the undersigned releases Nick Young from any and all liability for any failure to identify any medical condition or disease. It is understood and agreed that this is not the purpose of his services.

 

REFUND POLICY: Service Fees are Non-Refundable, with the exception of, that if a single consultation appointment is to be cancelled no later than 48 hours prior to the start time booked of the service consultation, a refund for the single (one-time) consultation service, minus the credit/debit card transaction fees, will be designated. However, once service fee is paid for the Heilen+plus Program or any One-Month, Two-Month or Three-Month Program, service will begin for the duration of the arrangement. There are typically no refunds for the Heilen+plus Program or any 90-Day Program unless the package is unopened and returned without engagement with your practitioner, then a refund may commence upon request if the practitioner agrees.

 

Lab Testing: If you make a mistake collecting saliva or urine specimens, etc., a replacement test kit may be provided to you, however there is no guarantee as this falls under the laboratory replacement kit policy and may vary from each individual laboratory. Please call the office before discarding any specimens collected. Once test kits are in your possession, they may not be returned and no refunds are allowed for any test kits. Our fee includes a nutritional interpretation of test results and recommendations for changes in lifestyle including diet, rest, exercise, stress reduction and supplementation. You should consult with your doctor before making any changes to your diet and exercise programs or taking any supplements.
